Fill out the form and drop it off along with your payment at the Fayette Police Department, Visual Element, or Fayette City Hall. Payments are due at the time of turning in the order form. You will be notified when to pickup your order. You may also mail in order forms & payments to The Fayette Police Department, PO Box 28, Fayette, IA 52142.

 

Proceeds will directly benefit the Fayette Police Dept. K9 program with equipment, food, and training.
